I have done a fairly quick and dirty rough draft of our marketing activity proposal proforma and included it on the N: drive
N:\PVC-SE\Library\Meetings\Library Marketing Group\Documents
You are most welcome to please provide feedback - it is a work in progress. Some of the wording may need a bit of tweaking, I think! I used the Application for Staff Development proforma as a model. Let me know what you think.
